The Care Coordinator plays a vital role within a multidisciplinary team, contributing to the effective delivery of the IPC program. This role involves close collaboration with all IPC team members and key stakeholders to support various aspects of the program, including referral processing, data collection, and stakeholder engagement.Experience RequirementsHigh level interpersonal and communication skills (verbal and written) and consultative skills reflected in an ability to engage and collaborate with, a wide range of service providers and community membersAbility to capture and share information using client record management systemsUnderstanding of chronic disease self-management principles and informationDemonstrated experience in working effectively with people with a chronic diseaseExperience in working with Aboriginal communities and the ability to provide culturally appropriate support.Qualification And Other RequirementsRelevant tertiary qualifications (primary health care, nursing, health promotion, public health, community development) or combination of relevant training and experience deemed to be appropriate.Drivers LicenceCurrent or willing to obtain a Working with Children's Check (WWCC)How To ApplyApplicants are requested to send a cover letter and resume to Sonder via our careers portal on our website.Feel free to send any enquiries to sondercareers@sonder.net.auAdvert closes: 18th of June 2025At Sonder, we strive to respond to all applicants.
