About the Role
Reporting directly to the Digital Value‑Based Coordinator, the Value‑Based Healthcare Case Manager will contribute to the development of valuable services for patients undergoing treatment. The primary objective is to improve the therapy experience and ensure optimal therapeutic adherence by defining and delivering personalized support plans and their corresponding clinical follow‑ups.
Key Responsibilities
Manage and monitor data from remote monitoring platforms of respiratory therapy devices (C‑PAP, pulmonary ventilators, etc.) to support therapeutic adherence.
Collaborate with the Digital VBHC Coordinator to design monitoring plans aimed at achieving the best therapeutic adherence.
Conduct personalized support programs (phone and remote) for patients undergoing remote monitoring.
Enter, monitor, and report data from digital platforms into dedicated IT tools.
Develop and administer ad‑hoc educational reinforcement programs aligned with each patient’s personalized support plan.
Requirements
Degree in a scientific or healthcare field: Nursing or Physiotherapy with a Neuro and/or Respiratory specialization.
Maximum of 3 years of experience in a similar role within structured environments (Healthcare, Homecare, pharmaceutical, or medical device sectors).

Innovative and proactive thinker.
Fluency in English.
