Position Description
Gateway CHC, located in Tweed, Ontario, delivers primary health care through an interprofessional team emphasizing health promotion and illness prevention. We are seeking a Primary Health Care Nurse Practitioner (PHCNP) for a permanent 0.6 FTE position to provide holistic, client-centred care. The role partners with Gateway CHC and Central Hastings Family Health Team at Gilmour Medical Clinic, requiring the Nurse Practitioner to carry their own roster and act as the primary care provider.
Key Responsibilities

Conduct health assessments and physical examinations across the lifespan.
Use evidence-informed clinical reasoning to formulate diagnoses.
Collaborate with clients and care teams to establish and coordinate care plans.
Identify trends in client health needs and contribute to program planning and advocacy.
Maintain clear, accurate, and timely documentation (EMR, reports, requisitions).

Qualifications

Current registration with the College of Nurses of Ontario as a PHCNP.
Member of NPAO with current liability insurance.
Minimum 5 years' experience in a primary care role (community health preferred).
Completion of “Prescribing Narcotics and Controlled Substances” (NPAO preferred).
Proficiency in Microsoft Office and electronic medical record systems.
