Responsibilities

Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines.
Administer and order point‑of‑care and lab‑based testing as needed.
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
Collaborate with the primary care physician (PCP) on patient education and follow‑up, as necessary.
Provide services at the member’s place of residence, which can be in their homes or skilled nursing facilities.

Benefits of Working At Matrix

Flexibility – Visits are scheduled based on your availability, from Sunday – Saturday 8 am – 7 pm.
Competitive Pay – Earn income based on the number of assessments completed. Those completing 20‑100 visits/month earn $1,800‑$11,500. Pay ranges $90‑$115 per assessment.
Benefits – Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.

Additional Licensure – Support for obtaining additional state licensure and credentialing.
Full‑time (FT) Opportunities – PRN providers can transition to FT roles with a sign‑on bonus.

Qualifications
Skills & Experience That Shine

Master’s degree or commensurate experience and satisfactory completion of NP licensure.
Current NP licensure in state of practice, including prescription authority.
AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
Current BLS, ACLS, or CPR certification.
1 year of NP experience preferred; new graduates encouraged to apply.
Valid state driver’s license and proof of adequate automobile insurance.
Strong computer skills and familiarity with employee health/medical record software.
Excellent verbal and written communication skills.
