The CSC LPN works closely with the CSC RN and CSC RN Team leader.Performance Responsibilities and StandardsPromote continuity of care for referred clients:Document all necessary information provided by the referral source into the client’s electronic medical recordAnswer inquiry calls from clients, families, physician offices, community agencies and insurance companiesUpdate referral information as appropriatePerform coding review with proper trainingPromote quality assurance as it relates to home care planning:Participate in quality improvement activities related to home care planning and coordinationAdhere to quality standards including the confidentiality of client informationOther expectations:Promote positive relationships among referral sources, patients and familiesIdentify problems and report to CSC RN or CSC RN Team LeaderAttending meetings and conferences as requiredPerform all other duties as assignedRequirementsGraduate of an accredited licensed practical nursing program with current New York State Licensed Practical Nurse registrationSkills necessary include excellent computer, verbal, and written communication skillsExcellent organizational skills with the ability to prioritize multiple tasksExcellent customer service both internal and external customersAble to communicate effectively in English, both verbally and in writingPhysical RequirementsAbility to sit or stand at a desk 90% of the dayVisual/hearing ability sufficient to comprehend written or verbal communicationAbility to express self verbally and in writingAbility to use telephone and communicate over the phoneFrequently uses a laptop computer to enter orders and obtain product informationBloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluidsCompetitive Salary - This position is an H07 non-exempt position with a min-max rate of: $19.57- $29.35/hour401 with generous Employer matchOn-Site Gym (free for all employees)Tuition Reimbursement for BSN after 6 months of employmentExceptional work/life balance - no rotating shiftsPartially funded HSAEmployee Recognition PlatformPaid Time Off, Holidays, and Extended Sick LeaveShort/Long term DisabilityEmployee Assistance Program (EAP)Much More!________________________________________________About Nascentia HealthNascentia Health is leading the way in home care, post acute care and long-term community health.
